This agreement is between

and Ruff Cuts/Ruff “Inn” It dog groomery and daycare/boarding. The terms of this agreement include all agents, employees, officers, contractors, third parties and insurers of Ruff Cuts/Ruff "Inn" It (hereinafter referred to as Ruff Cuts/Ruff "Inn" It).

Ruff Cuts/Ruff "Inn" It is careful to provide appropriate and adequate supervision for all pets in their care. However, dogs in particular are prone to rough play and injuries can occur. Owner acknowledges that certain risk of injury or illness in the grooming, daycare and boarding setting do exist and Owner agrees to hold Ruff Cuts/Ruff "Inn" It harmless for all injuries resulting from pet(s) activities at Ruff Cuts/Ruff "Inn" It.

In consideration of receiving permission to utilize Ruff Cuts/Ruff “Inn” It dog groomery and boarding, I hereby RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE Ruff Cuts Grooming and Ruff “Inn” It boarding kennels, its agents, employees, officers and insurers from any and all liability, claim, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, sustained by me or to any property, including dog(s), belonging to me WHETHER CAUSED BY THE NEGLIGENCE of Ruff Cuts/Ruff “Inn” It dog groomery and boarding or otherwise, while participating in groomery or boarding activities, or while in, or upon the premises where the activity is being conducted.

I am the legal owner or authorized agent of this pet and I agree to pay for the daily boarding/daycare or groomery fees and for all ancillary services requested. I understand that I am responsible for keeping my pets record up to date including emergency contact, medical and any other critical information. To the best of my knowledge, my dog is in good physical condition and I am not aware of any physical infirmity which would place my dog or myself at risk in participating in grooming or boarding. I acknowledge that there are risks involved in dogs participating in playtime activities, including but not limited to illness, bites, scratches, and death. I acknowledge that vaccines do not cover every strain of communicable illness and even vaccinated pets are susceptible to illness. I accept full responsibility for any damage or injury to persons, property or animals arising out of my dog’s participation in play activities, use of the grounds or facility and for the actions and conduct of my dogs. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISK OF LOSS, PROPERTY DAMAGE OR INJURY, INCLUDING DEATH, that may be sustained by me or my dog, or any loss or damage to property owned by me, as a result of being engaged in such activity WHETHER CAUSED BY THE NEGLIGENCE of Ruff Cuts/Ruff “Inn” It dog groomery and boarding or otherwise. I further hereby AGREE TO INDEMNIFY AND HOLD HARMLESS Ruff Cuts/Ruff “Inn” It from any loss, liability, damage or costs, including court costs and attorney’s fees, that may incur due to you or your dogs participation in said activity, WHETHER CAUSED BY THE NEGLIGENCE of Ruff Cuts/Ruff “Inn” It or otherwise.

It is my express intent that this Waiver of Liability and Hold Harmless Agreement shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representatives, if I am deceased, and shall be deemed as a RELEASE, WAIVER, DISCHARGE AND DEFEND, INDEMNIFY, HOLD HARMLESS AND COVENANT NOT TO SUE the Ruff Cuts/Ruff”Inn” It. This duty of defense and indemnification shall include any attorneys’ fees and costs incurred by Ruff Cuts/Ruff “Inn” It in responding to or defending any action brought by any third party in connection with the activity. I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be construed in accordance with the laws of the State of Washington. In signing this release, I acknowledge and represent that I have read and understand it and sign it voluntarily; I am at least eighteen (18) years of age and fully competent; and I execute this Release for full, adequate and complete consideration fully intending to be bound by the same.

Owner hereby gives absolute right and permission to Ruff Cuts/Ruff “Inn” It to use my likeness or that of my pet for any purpose whatsoever, including, but not limited to, to publish, broadcast and copyright my voice and video recording, name, picture, and likeness or any material based upon or derived therefrom, or to refrain from so doing, in any manner or media whatsoever for all purposes whatsoever for any and all media and related advertising and promotion thereof. I agree that any voice and video recording, picture or likeness of me or my pet, or anything derived therefrom created at Ruff Cuts/Ruff “Inn” It. If I should receive any print, negative, or other copy, I shall not authorize its use by anyone else. I shall have no right of approval, no claim to additional compensation, and no claim (including, without limitation, claims based upon invasion of privacy, defamation or right of publicity) arising out of any such use, alteration, distortion, or illusionary effect or other use in any composite form. I agree that this release does not in any way conflict with any existing commitment on my part.

Owner agrees that should a dispute arise from any action involving pet, Ruff Cuts/Ruff “Inn” It reserves the right to release Owner’s name, address, phone number or other pertinent information to those involved, to include, but not limited to, legal entities, animal control and/or law enforcement.

Owner further understands and expressly agrees that each and every of the foregoing provisions contained above shall be in full force and effect and shall apply to each and every occasion in which Owner’s dog(s) is boarded, groomed or other services at Ruff Cuts/Ruff “Inn” It. This liability waiver applies to all my current and future pets and all co-owners of my past, present and future pets. I accept responsibility to educate co-owners or agents of this agreement.

I HAVE READ THIS WAIVER OF LIABILITY AND FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

I hereby authorize Ruff Cuts/Ruff “lnn” lt boarding kennels to board/care for my pet(s). I am the legal owner or authorized agent of this pet and I agree to pay the daily boarding/daycare fee and for all ancillary services requested. I understand that I am responsible for keeping my pets record up to date including Emergency Contact, vaccinations, medical and any other critical information. I agree to immediately contact Ruff "Inn" It or use my online portal to update this information when changes occur. If emergency care should become necessary for my pet, I authorize Ruff- “Inn”-lt boarding kennels to obtain veterinarian services and agree to accept full financial responsibility. I agree to adhere to the pickup and drop off policies provided in my booking confirmation and understand that I may be charged for late pick up or drop off. Ruff “Inn” It reserves the right to decline services to aggressive or otherwise dangerous dogs, dogs that are not up to date on immunizations, arrive at the facility with fleas or ticks or other unsafe situations. As the owner/authorized agent of the pet I understand that at any time while at our facility a situation arises where my dog(s) becomes a threat to other dogs or staff, they may require immediate pick up. If the owner/agent isn’t available, Ruff “Inn” It will attempt to contact the listed emergency contact. In consideration of Ruff Cuts/Ruff-Inn-It boarding kennels, accepting my pet for boarding, I do hereby release, discharge and waive claims, demands, and /or actions against Ruff Cuts/Ruff-“lnn”-lt boarding kennels, its agents, employees, officers and insurers arising from or relating to injury, illness or death that may occur during the period of boarding in accordance with the attached waiver.
